You may access the ACPF Schedule of Support and Statement of Intent by logging in to the "Members Only" section of the American College of Phlebology website and clicking "Foundation Donation Forms" on the bottom of the menu on the right. If you are not a member of the ACP, please contact us to obtain the documents.
